What is Leptospirosis?

Leptospirosis is a zoonotic bacterial disease caused by pathogenic Leptospira species.

Transmission

The disease spreads through direct or indirect contact with water or soil contaminated with the urine of infected animals, particularly:

  • Rodents (major reservoir)
  • Cattle
  • Pigs
  • Dogs
  • Other domestic animals

Humans become infected when the bacteria enter the body through:

  • Cuts or abrasions on the skin
  • Mucous membranes (eyes, nose, mouth)
  • Prolonged exposure to contaminated floodwater or soil

Note: Human-to-human transmission is extremely rare.


Why Does Kerala Report a High Disease Burden?

Kerala has reported endemic leptospirosis since 1989 and currently records the highest mortality among communicable diseases in the State.

Current Context: In July 2026, leptospirosis claimed 50 lives, highlighting its growing public health concern.


1. Favourable Climatic Conditions

Kerala’s tropical climate provides ideal conditions for the survival of Leptospira bacteria.

Contributing Factors

  • Heavy monsoon rainfall
  • Frequent floods
  • Waterlogging
  • High humidity
  • Warm temperatures

These conditions allow the bacteria to survive for longer periods in contaminated water and moist soil, increasing the risk of human exposure.


2. Abundance of Animal Reservoirs

Kerala has a high density of:

  • Rodents
  • Domestic livestock
  • Companion animals

The close proximity of these animals to human settlements increases environmental contamination through infected urine.

Rodents remain the principal reservoir responsible for sustaining disease transmission.


3. Occupational Exposure

Certain occupations involve frequent contact with contaminated water and soil, increasing infection risk.

High-Risk Groups

  • Farmers
  • Dairy workers
  • Sanitation workers
  • Canal and drainage cleaners
  • Construction workers
  • Migrant labourers

Emerging Urban Risk

Urban cases are increasingly reported among people exposed during:

  • Gardening
  • Cleaning flooded premises
  • Construction activities
  • Recreational exposure to stagnant water

4. Better Disease Surveillance

Kerala reports a higher disease burden partly because of its strong public health surveillance system.

The State has:

  • Extensive diagnostic facilities
  • Systematic disease surveillance
  • Better reporting of confirmed and probable cases

Thus, Kerala’s higher numbers partly reflect better detection, not merely higher transmission.


Clinical Features

Leptospirosis initially presents with non-specific symptoms, making early diagnosis difficult.

Common Symptoms

  • Fever
  • Severe muscle pain (myalgia)
  • Headache
  • Conjunctival suffusion (red eyes without discharge)
  • Nausea and vomiting
  • Abdominal pain
  • Diarrhoea

These symptoms often resemble other tropical diseases such as:

  • Dengue
  • Malaria
  • Scrub typhus
  • Viral fever

Severe Disease

Approximately 5–10% of infected individuals develop severe leptospirosis.

Major Complications

  • Pulmonary haemorrhage
  • Acute kidney injury (AKI)
  • Liver dysfunction
  • Meningitis
  • Septic shock
  • Multi-organ failure

These complications significantly increase mortality.


Prevention and Control

Kerala follows a preventive strategy combining chemoprophylaxis, surveillance, and public awareness.

Chemoprophylaxis

The State recommends Doxycycline (200 mg twice weekly) for high-risk occupational groups during the monsoon season.

  • Inhibits bacterial multiplication
  • Reduces disease severity
  • Lowers the risk of complications

Public Health Measures

  • Rodent control
  • Proper waste management
  • Safe drainage systems
  • Use of protective footwear and gloves

  • Enhancing rodent control measures
  • Promoting occupational safety for high-risk workers
  • Increasing public awareness during monsoon seasons
  • Ensuring timely chemoprophylaxis and early treatment
